As a special dispensation, IAF seminar attendees can practice at Hombu Dojo free of charge for the following classes:
Dates: 18th (Tue), 19th (Wed), 20th (Thu), 21st (Fri)
(The Dojo will be closed on 17th (Mon), 22nd (Sat) & 23rd (Sun))
Times: Regular Class: 06:30-07:30 and 19:00-20:00
Beginners’ Class: 07:00-08:00 (Basically for white belt, under shodan)
*When you arrive at Hombu Dojo, please show your IAF seminar ID at the front desk

Please submit the application form, though you arrange your accommodation by yourself.
(2) After you sent the application form, you will receive the notification of receipt and the invoice by e-mail.

With my participation in 11th International Aikido Seminar, I agree to the following conditions. I confirm as a representative of my group that all of my members agree to the same conditions.
- I swear that I take responsibility for my health.
- I understand that participation is solely my own responsibility and I swear that I assume all risks of injury, illness and other accident.
- I agree to release all instructors, staff members, employees and agents of the sponsoring organizations and facilities from any and all liability, for all activities both during and outside of training sessions for the course of the seminar.
- I agree to take all responsibilities for property loss and breakage of my possessions and our group will secure our property ourselves. I swear that I release the organization from any and all liability.
- I agree to sign up for the Overseas Travel Insurance Policy (1,000JPY)* made available by IAF.
(*The above insurance will cover only for injury during the seminar, from 17th till 24th Sep. Therefore we suggest taking out travel insurance by yourself for duration of your trip.)

3. 申込方法
（１）下記誓約書に同意の上、申込書に必要事項を記載し、下記アドレスまでemailして下さい。

※ご自身で宿泊の手配をされる場合でも、この申込用紙をご提出下さい。
（２）後日、申込受領の通知と請求書がemailにて返信されてきたら、受付完了です。
（３）請求書に基づき送金完了後、申込受領書兼領収メールが送られてきたら申込完了です。
（４）申込受領書兼領収メールをチェックイン当日、係員にご提示下さい。

5. さよならパーティの参加費は3000円です。
誓約書
私は第11回大会国際合気道大会講習会に参加するに当たり、下記事項に同意します。また同伴参加者を代表して参加者全員が、下記項目に同意したことを認めます。
1. 私は、自分の健康に責任を持つことを約束します。
2. 私は本講習会への参加は、私個人の責任において行い、この期間中の怪我、病気、その他の事故に関する
責任は全て自分で取ります。
3.私は、稽古中及び稽古以外のすべての活動において起こった怪我、その他の自己については、上記講習会
における指導者、主催者側の職員とその全ての関係者に関して、責任追及及び、損害賠償の請求を一切し
ないことに同意します。
4.私は、本大会中の所持品の盗難、紛失物、破損に関しては一切の責任を持ち、各自及び団体毎の管理を心掛
け、主催者側にそれらの責任を一切追及しないことを誓います。
5.私は、IAF規定の海外旅行保険*（1,000円）に加入することに同意します。
（*本保険は9/17～24の傷害のみが対象となるため、ご自身で総合旅行保険への加入をお勧めします。）
